Distinctive characteristics of this form:

top right box has the words "AFFIX STAMPS HERE";
some of these forms/vouchers were probably made available in pad form (see serrations along edge in AW-RO-7G) with the voucher uppermost;
the main distinctions between the two varieties are:
7A: three horizontal and two vertical thick lines;
7C: three horizontal and no vertical thick lines;
7D: no thick lines.
